Transition Coordination Services Options Counseling

Transition Coordination Services is a program designed to help facilitate clients who want to transition from nursing or long-term care facilities to the community using home and community-based (HCBS) services and supports. The Boulder County Area Agency on Aging (AAA) is the local contact agency (LCA) for Transition Coordination Services Options Counseling. This is the first step in the Transition Coordination Services process and helps clients explore their options of moving back to the community from the nursing facility.

Any resident living in a skilled nursing facility, under their Long Term Care Medicaid benefit, can request an Options Counseling meeting.
